To be completed in entirety and signed by parent or legal guardian. All campers must be registered, including adults. This form must be completed by EACH camper.
Ages 11 and under MUST BE accompanied by a parent or guardian. The guardian must be 20 years old.
Haven of Hope's insurance is a secondary insurance only. We fully expect your primary insurance to cover all costs should injury occur. I have read the attached rules and this registration form. I intend to fully abide by all rules and subject myself to the leadership of Haven of Hope Church. I release all liability from Haven of Hope Church and/or Camp Chinquapin.
